Biceps Tendon Injury

The biceps tendons connect the muscles in the front part of your upper arm to the bones in your elbow and your shoulder. There are two biceps tendons in the shoulder called the long head of biceps and the short head of biceps.

There is one biceps tendon in the elbow.  These tendons can be torn or ruptured in an impact sport like rugby or they can be slowly damaged over a long time. Tears at the elbow are uncommon and it is more often that the shoulder insertion is ruptured or torn.

Causes of Biceps Tendon Injury

The shoulder or proximal biceps tendons can be injured in an impact sport such as rugby from making a partial tackle with an outstretched arm. Tendonitis can also be caused by repetitive motion so swimmers and tennis players and golfers can develop tendonitis in the shoulder.

Depending on the site of the injury, shoulder or elbow, you may have sudden severe pain and feel a ‘pop’ when the tendon tears. Bruising may appear on the upper arm or forearm near the elbow and the shoulder or elbow may feel weak.  Sometimes the biceps muscle bulges like ‘Popeye’ and some of the movements of the arm are limited.

Initially treat with cold packs and ice and then seek an accurate diagnosis to see if the tendon is ruptured or torn. You will need a thorough assessment and possibly further investigations to determine whether surgical intervention is required to re attach the tendon. If you have surgery you will need physiotherapy and will need to follow a graduated exercise programme to aid full recovery.  Tendonitis can be treated with corticosteroid injection and there is good success with these injections if the tendonitis has not resolved with conventional therapies such as physiotherapy.
